Instructions:
First print the Passport Cover and set it aside. (You may wish to use card stock.) Then, print Pt I, turn the pages over, and print Pt II on the other side. Cut horizontally on the dashed line and remove the one blank page. Put the top and bottom parts together, add the cover, fold over and staple in the seam.
Copies of the Passport were distributed to many local sites, made possible with funding from Credit Data Services, Inc. Fund and the Western Massachusetts Community Fund.
